Person Centred Care

At Edmund Street Activity Centre, we see the person before the disability. Yes, it is true that meeting people's health and social care needs is fundamental to providing a safe and effective service, but this cannot be truly achieved without acknowledging the massive part that happiness plays in both the physical and emotional wellbeing of every individual.

“Everyone Deserves To Be Seen Beyond Their Disability”

PERSON-CENTRED

Finding the Right Balance

Each person that joins our service completes an assessment with the manager or one of our team leaders. We call this assessment an ‘Assessment of Strengths, Needs and Risks’. Either yourself and/or your family member or representative can complete this assessment, and the aims of this are to identify who you are as an individual, what care and support you needs you have, and how we will balance respecting your choices and preferences whilst keeping you safe.

The information that you share with us is then used to develop your plan of care and support. This document is used by the support team to get to know your likes and dislikes. How to meet your care and support needs in ways that you are happy with, and how to minimise risks to your health, safety and emotional wellbeing. Every activity that is provided is inclusive and is delivered in a person-centred manner.

COME VISIT US!

Bespoke Care and Support

We encourage you and the people that are important to you to be as involved as possible when designing your care and support plan. We hold you and the people that are important to you in very high regard and respect both yours and their expertise in the knowledge that you and they have about how to meet your needs in the ways that you prefer. The only expectations are from us, as you or the person that you are representing have full autonomy over the care and support that you receive.

We care about the people and the things that are important to those using our service. We can be approached anytime for advice and support, and we will always listen and act if there is something that we can do to improve our service for you, your loved one or the person you are supporting.
